Habitual intemperance in the use of alcohol or habitual substance abuse, use of controlled substances or prescription-only drugs except if provided by a physician for use during a prescribed lawful course or treatment, providing any controlled substances or prescription-only drug for other than accepted therapeutic purposes, Licensee failed to obtain informed consent from patient prior to the performance of any surgical procedure. Licensee failed or refused to maintain adequate records on patient. Licensee engaged in conduct or a practice which was harmful or dangerous to the health of patient.
